Petunjuk Pendaftaran Online

一、本次招生網路報名日期為即日起 至 115年10月23日止， 系統將於115年10月23日 下午5時 關閉。逾時將無法上線受理報名。本人將依限完成網路報名資料填載，並於確認各項報名資料填妥無誤後列印報名表件， 至遲於115年10月23日前， 以掛號郵寄嘉南藥理大學招生委員會，郵戳為憑，逾期或表件不全者，同意本校註銷報名資格。
(嘉南藥理大學並未委託或授權其他單位或個人處理招生相關業務或收取任何費用。)

一、The online application period for this admission begins from now until 10/23, 2026. The online application system will close at 5:00 p.m. on 10/23, 2026. Applications submitted after the deadline will not be accepted. I shall complete the online application within the specified period. After confirming that all application information has been completed correctly, I shall print out the application documents and send them by registered mail to the Admissions Committee of Chia Nan University of Pharmacy and Science no later than 10/23, 2026. The postmark date shall serve as proof of timely submission. I understand and agree that the University may cancel my application eligibility if the application is submitted after the deadline or if the required documents are incomplete.
(Chia Nan University of Pharmacy and Science has not commissioned or authorized any other organization or individual to handle admission-related matters or collect any fees on its behalf.)

一、Thời gian báo danh trực tuyến từ nay đếnngày 23 tháng 10 năm 2026. Hệ thống báo danh sẽ đóng vào lúc 17h00 ngày 23 tháng 10 năm 2026.Quá thời gian trên sẽ không tiếp nhận xử lí hồ sơ báo danh trực tuyến. Người báo danh cần điền thông tin báo danh trực tuyến theo đúng thời hạn và sau khi xác nhận rằng tất cả thông tin báo danh là chính xác, người báo danh cần phải in mẫu đăng ký và gửi bằng bưu điện đến Hội đồng tuyển sinh của Đại học Dược Gia Nam trước ngày 23 tháng 10 năm 2026. Ngày gửi bưu điện(ngày đóng dấu trên bì thư) sẽ được lấy làm chuẩn. Nếu hồ sơ báo danh quá hạn hoặc không đầy đủ, nhà trường sẽ hủy bỏ tư cách báo danh của thí sinh.

一、Periode pendaftaran online untuk penerimaan mahasiswa ini berlangsung mulai sekarang hingga 10/23, 2026. Sistem pendaftaran online akan ditutup pada pukul 17.00 tanggal 10/23, 2026. Pendaftaran yang dilakukan setelah batas waktu tersebut tidak akan diterima. Saya akan melengkapi seluruh informasi pendaftaran online sesuai dengan batas waktu yang ditentukan. Setelah memastikan bahwa seluruh informasi pendaftaran telah diisi dengan benar dan lengkap, saya akan mencetak dokumen pendaftaran dan mengirimkannya melalui pos tercatat kepada Panitia Penerimaan Mahasiswa Chia Nan University of Pharmacy and Science paling lambat 10/23, 2026. Tanggal cap pos akan digunakan sebagai bukti tanggal pengiriman. Apabila dokumen dikirim melewati batas waktu atau tidak lengkap, saya menyetujui bahwa pihak Universitas berhak membatalkan kelayakan pendaftaran saya.
(Chia Nan University of Pharmacy and Science tidak pernah menunjuk atau memberikan wewenang kepada organisasi maupun individu lain untuk menangani urusan penerimaan mahasiswa atau memungut biaya apa pun.)

二、本人已詳閱應考須知，明瞭招生相關規定(簡章下載)，茲以網路報名嘉南藥理大學2027年國際專修部申請入學招生，將詳實填寫並寄送相關資料以供嘉南藥理大學招生委員會審查，所填資料如經查證不實者，其責任由本人自負。

二、I have carefully read the application instructions and fully understand the relevant admission regulations (Download Admission Prospectus). I hereby apply online for admission to the 2027 International Foundation Program of Chia Nan University of Pharmacy and Science. I shall truthfully complete and submit all required documents for review by the Admissions Committee of Chia Nan University of Pharmacy and Science. I accept full responsibility for any false or inaccurate information found in my application.

二、Tôi đã đọc kỹ hướng dẫn chi tiết và hiểu các quy định tuyển sinh liên quan (tải xuống tài liệu tuyển sinh). Tôi đã báo danh trực tuyến lớp chuyên ban sinh viên quốc tế hệ học và thực tập tại doanh nghiệp học kỳ 2 năm học 2027 tại Đại học Dược Gia Nam. Vui lòng điền chi tiết các thông tin liên quan và gửi đến Hội đồng tuyển sinh của Đại học Dược Gia Nam để xét duyệt. Nếu phát hiện thông tin điền sai sự thật, tôi sẽ hoàn toàn chịu trách nhiệm.

二、Saya telah membaca dengan saksama petunjuk pendaftaran dan memahami seluruh peraturan terkait penerimaan mahasiswa (Unduh Panduan Penerimaan). Dengan ini saya mengajukan pendaftaran online untuk Program Internasional Tahun 2027 di Chia Nan University of Pharmacy and Science. Saya akan mengisi seluruh informasi dengan benar dan lengkap serta mengirimkan dokumen yang diperlukan untuk diperiksa oleh Panitia Penerimaan Mahasiswa Chia Nan University of Pharmacy and Science. Apabila ditemukan informasi yang tidak benar atau tidak sesuai dengan fakta, saya bersedia bertanggung jawab sepenuhnya.

三、關於本人的基本資料及報名資料受到隱私權及個人資料保護法之法令保護與規範。

三、Thông tin cơ bản và thông tin báo danh cá nhân của tôi được bảo vệ và quản lý bởi Đạo luật bảo vệ thông tin cá nhân và quyền riêng tư.

三、Data pribadi dan informasi pendaftaran saya dilindungi dan diatur berdasarkan peraturan perundang-undangan yang berlaku mengenai privasi dan perlindungan data pribadi.

四、報名費:免繳。

四、Application Fee: Free of charge.

四、Biaya Pendaftaran: Gratis.

五、報名相關資料經完成填寫、列印報名書表寄遞後，非經同意不得要求更改，兩者如不相符時，以所寄達的書面報名資料為準。

五、After the application information has been completed and the printed application documents have been mailed, no changes may be requested without prior approval. In the event of any discrepancy between the online information and the printed documents submitted by mail, the printed application documents received by the University shall prevail.

五、Sau khi hoàn thành điền dữ liệu báo danh liên quan, in đơn báo danh và gửi qua đường bưu điện, không được phép thay đổi nếu không có sự đồng ý. Nếu cả hai thông tin này không khớp nhau thì thông tin báo danh bằng văn bản đã gửi được lấy làm chuẩn.

五、Setelah informasi pendaftaran selesai diisi dan dokumen pendaftaran yang telah dicetak dikirim melalui pos, perubahan tidak dapat dilakukan tanpa persetujuan terlebih dahulu. Apabila terdapat perbedaan antara informasi pendaftaran online dan dokumen tertulis yang dikirimkan, maka dokumen tertulis yang diterima oleh pihak Universitas akan dijadikan sebagai acuan.

六、本人有責任維持帳號及密碼的機密安全。
